What is One Chart | PATIENT? Return to Top

One Chart | PATIENT is a secure, online web portal that offers personalized access to parts of your medical records. With One Chart, you will be able to:

  • Review your medical records.
  • Review your familyís medical records, including immunizations.
  • Review tests results.
  • Review billing and insurance information.
  • Request an appointment.
  • Ask your doctor a question online.

How do I communicate my medical concerns? Return to Top

One Chart | PATIENT is not used to communicate serious medical concerns to your doctor. Please schedule an appointment with your doctor to discuss immediate medical concerns or problems. If you have general questions about your condition or health that you do not feel warrant a visit to your provider, you may now submit your questions online. A response will be provided to you within one business day by your health care provider.

If you are experiencing a medical emergency, call 911 immediately.

Can I schedule an appointment with my doctor online? Return to Top

You may request an appointment with your health care provider online. You will indicate which days of the week and times of the day work best for you. Once the office staff receives your appointment request, you will receive a follow-up phone call or email confirming your appointment date and time.

Why doesn't all of my health information show up in my One Chart | PATIENT record online? Return to Top

Your health summary contains a record of health information that has been released by doctors affiliated with The Nebraska Medical Center, the University of Nebraska Medical Center, Bellevue Medical Center, UNMC Physicians, Clarkson Family Medicine or private offices that use One Chart | PATIENT. You can discuss this with your primary care physician during your next visit. Your health records from other offices or hospitals not affiliated with One Chart will not be available in One Chart. Only records after May 1, 2012 for physician clinic visits and after Aug. 4, 2012 for hospital services will be accessible through One Chart.

Why arenít all of my test results showing up in my One Chart | PATIENT account? Return to Top

Because certain test results require provider interpretation, only specific test results are viewable online through One Chart | PATIENT. The tests available for viewing are continually reviewed and updated allowing us to share as much information as possible with you.

Iíve seen many providers over the years. Which ones can I make an appointment with? Return to Top

You can request an appointment with any physician that you have seen within the last two years who is affiliated with One Chart. These physicians include doctors at The Nebraska Medical Center, the University of Nebraska Medical Center, Bellevue Medical Center, UNMC Physicians, Clarkson Family Medicine or any private practice physician using One Chart | PATIENT.

You may send brief, electronic, health-related messages to any physician you have seen within the last six months.

My preventive care recommendations donít seem right to me, what do I need to do? Return to Top

The preventive care recommendations provided are based on your age, gender and diagnosis, if applicable. Please discuss this information with your physician during your next visit to determine if modifications may need to be made to your preventive care plan.

How do I get a complete set of my medical records? Return to Top

You can obtain your medical records by contacting the Medical Records Department at 402-559-4024.

How may I view the health record of another adult, family member or minor online in One Chart | PATIENT? Return to Top

The One Chart | PATIENT Proxy option allows a parent or guardian to log into their personal One Chart | PATIENT account and then connect to information regarding another adult, family member or minor.

In order to access information for another person, you must first have an active One Chart | PATIENT account.

If you already have an active One Chart | PATIENT account, you will need to fill out the applicable One Chart | PATIENT Proxy Access form. You can access the form by logging into your One Chart | PATIENT account and clicking on "My Familyís Records."

These include:

  • Proxy form to provide another adult with online access to One Chart | PATIENT
  • Proxy form to provide online access to One Chart | PATIENT to a guardian of a minor
  • Proxy form to provide online access to your childís One Chart | PATIENT account (legal parent)

After the proxy form has been filled out, please mail the form to:

Please allow 14 business days to process the proxy form. After 14 days, you should see a ďproxy relationshipĒ in your One Chart account so that you can access the health record of the approved adult, family member, or minor. Once a child turns 19, they may get their own account.

I completed the proxy forms to access the health records of another, but I am not able to see their information in my One Chart | PATIENT account. Return to Top

Please allow 14 business days for your proxy request to be processed. To check on the status of your request, contact customer service at OneChartPatient@NebraskaMed.com or call 402-559-8158. Please include the name and date of birth for each person for whom you requested proxy access.
